We want every patient to be able to use this website, complete our forms, and communicate with our office.
Our commitment
California Center for Behavioral Health is committed to making our website and our services accessible to people with disabilities. We aim for this site to meet the Web Content Accessibility Guidelines (WCAG) 2.2 at Level AA, and we review it for accessibility as we make changes.
Requesting an accommodation
Most visits are conducted by telehealth. If you need an accommodation to complete our forms, attend a telehealth visit, or communicate with our office, please tell us. We can help with, for example:
- Completing intake and consent forms by phone with staff assistance, or on paper by fax
- Forms and notices in an alternate format, such as large print or plain text
- A sign language interpreter or other communication aid for your telehealth visit, at no cost to you
- Captioning, or an alternative way to connect if the video platform is difficult for you to use

Online forms and PDFs
Our online forms are the most accessible way to complete your paperwork: they work with screen readers, can be completed with a keyboard alone, and announce any errors. The downloadable PDF versions are provided as a print-and-fax alternative and may not be fully accessible to assistive technology. If you need a PDF form in a different format, please call us and we will provide one.
Tell us about a problem
If any part of this website is difficult for you to use, or you find content that is not accessible, please let us know by calling (925) 289-9022. Tell us the page you were on and what happened. We will work with you to provide the information or service you need another way.
Assistant chat
The chat assistant on our home page can be opened, used, and closed with a keyboard, and its replies are announced to screen readers. It is intended for existing patients to request follow-up appointments or medication refills. It is not monitored for emergencies.
